For your convenience, and as a time saver at your office visit, please find any necessary forms below. Please download and complete these forms and bring them to your next scheduled appointment.

Family and Medical Leave Act

Valley OBGYN is here to assist you with the completion of your disability or FMLA forms. We have trained staff who work closely with the providers to complete and submit your medical leave paperwork to either your employer or disability company.

Completion of disability or FMLA forms takes approximately 7-10 days. Your paperwork can be submitted to our office by email, fax, mail, or you can drop it at the front desk of either of our locations. A $45 form completion fee will be collect at the time you pick up your completed form(s).

If you are needing FMLA paperwork completed by our Providers, the following information is required from you in order for you to complete your request:

  • Your portion of the form must be completed and accurate
  • You have signed the form
  • Please state how much time you are requesting off
  • If the paperwork is for a spouse or support person, please make sure their complete information is filled in and your first and last name and date of birth is also with the form
  • If you are wanting our staff to fax or email your forms to your employer or disability company, please make sure you have signed the release of information attached, OR if there is not one with your paperwork, please sign one of our Medical Records Release Forms

Washington State FMLA Forms

Please note that for Washington State FMLA, we cannot fill out the forms until after the birth of the baby, as the state will not accept them until then.

Duration of Leave:

  • For vaginal delivery, you are eligible for 16 weeks off.
  • For cesarean delivery, it’s 18 weeks.

Form Completion:

  • The form will initially be filled out for either six or eight weeks.
  • Once these initial weeks are up, you can log onto your SAW account to add on the additional 10 weeks of bonding time, which you can use all at once or throughout the year.
  • Your spouse or support person will receive 12 weeks to use all at once or throughout the year as well.

Preparation:

  • We encourage you to create a SAW account on the WA State FMLA site before delivery to help speed up the process for you once you deliver.

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. We ask you to give us 24 hours advanced notice, providing an opportunity for another patient to be seen.

Depending on your insurance company, some services will require a copay and some will go towards your deductible. Copays, coinsurance and/or deductibles are collected at time of service.

For after hours (nights, weekends, Holidays) please call (509) 924-1990 and you will be directed to our answering service.

Your pharmacy can fax a refill request to our office at (509) 232-3059. You can also request prescription refills through the patient portal.

Unless there is a medical problem, you should have your first pelvic exam around the age of 21 or when you first become sexually active.

Yes! You can have a pap smear during your cycle as long as the flow is light and not heavy.

You may not need a pap every year, but it is still important for your health to have a yearly pelvic exam, for cervical cancer and breast health exam.
